Volunteer Verification Form

To submit your Volunteer Service Documentation, you will need to:

Download and complete the Volunteer Verification Form.
Sign and allow your Contact Person to sign the document as well.
Scan the completed document and save it to your computer in a location that is easily accessible such as your "Desktop" or "My Documents".
Using the field below, browse to the location where you previously saved the completed document.
Once found, double-click the document to attach it.
After attaching the document, click the Submit button to complete the Volunteer Service Documentation.
