Volunteer Verification Form

To submit your Volunteer Service Documentation, you will need to:

  1. Download and complete the Volunteer Verification Form.
  2. Sign and allow your Contact Person to sign the document as well.
  3. Scan the completed document and save it to your computer in a location that is easily accessible such as your "Desktop" or "My Documents".
  4. Using the field below, browse to the location where you previously saved the completed document.
  5. Once found, double-click the document to attach it.
  6. After attaching the document, click the Submit button to complete the Volunteer Service Documentation.
