FACILITY LICENSE APPLICATION
IMPORTANT INFORMATION TO REVIEW BEFORE SUBMITTING YOUR APPLICATION

·        A non-refundable $150 USD fee is required upon completion of this License Application.

·        Only submit one License Application per facility address. Do not use your browser's back button during the application process.

·        Internet Explorer is not supported.  Please use a browser like Chrome, Edge or Firefox to complete this form.

 

A new License Application is required for:

·        Any new facility seeking R2 Certification, including campus locations

·        A new address for a facility move

·        Facilities reentering the R2 Certification program after the previous certificate at the same address has been withdrawn


A new License Application is NOT required for:

·        Company name or facility address revisions

·        Transferring an active R2 Certificate to a new Certification Body

·        Requesting copies of an existing signed License Agreement, License Acknowledgement, or invoice
