REQUIRED INFORMATION

Kindly complete, sign and submit these forms by 15 July 2022. Receipt of these materials is required for your acceptance to the AFRE Fellowship. Please email any questions or concerns to afreinfo@atlanticfellows.org

YOUR CONTACT INFORMATION AND EMERGENCY CONTACT

As a member of our community, we need up-to-date contact information so that we can communicate effectively with you. AFRE staff must be informed of changes in your contact information (including phone number, mailing address, and a secondary email address). You also are required to notify AFRE staff of any changes in the name(s) and address(es) of the person(s) to be notified in the event of an emergency.

Once you graduate from the fellowship, you will be asked for updated contact information to assist us with community building among our fellowship community, as well as in monitoring your fellowship project implementation and impact. You also may be asked for other information at various times by AFRE or the Atlantic Institute.

FINANCIAL FORMS FOR SCHOLARSHIP AND REIMBURSEMENTS


The following information is required to enter you into the AFRE database at Columbia University for US-based Fellows and at the Nelson Mandela Foundation for SA-based Fellows.


FOR US-BASED FELLOWS:
Required Forms:

  • Set Up Form for Fellow Scholarship & Reimbursements
  • W9 Form
  • If you are requesting that your scholarship be paid to an organisation, instead of you personally, please be sure to have ready the organisation's current 501c3 designation letter, W9 and the main finance person's contact details. This is crucial to the timely payment of your first scholarship allocation.

Process:

Please note that you will be required to upload these documents on page 6 of this form. Once we have received these forms, Meredith Shin will contact you regarding the process. You will have the option to receive payments via direct deposit.

FOR SA-BASED FELLOWS:
Required Forms:

  • Set Up Form for Fellow Scholarship & Reimbursements
  • NMF Contract Letter
  • If you are requesting that your scholarship be paid to an organisation, instead of you personally, please be sure to have ready the organisation's BBBEE certificate, tax clearance document and their registration document. This is crucial to the timely payment of your first scholarship allocation.
Process:
Please note that you will be required to upload these documents on page 6 of this form. Once we have received these forms, Ntombifuthi Mbongo will contact you regarding the process. Your payment will be made through EFT.

NEEDS ASSESSMENT QUESTIONNAIRE


Fellows are required to complete the following fellowship needs assessment questionnaire.

1. AFRE will be sending all Fellows a care package that will include contents aimed at supporting your participation in enhancing your experience of the Virtual Retreats. To help us avoid purchasing items or equipment you may not need, please let us know which items you would like to receive.

COMMUNICATIONS CONSENT FORM

The Atlantic Fellows for Racial Equity (“AFRE”) develops content to build the public profile of our  Fellows and increase the visibility and reach of our programme. This includes collecting and  publishing professional biographies and headshots as well as photography, videos, social media,  and written content that reflect on the fellowship experience. As an Atlantic Fellow for Racial  Equity, you are agreeing to the following terms and conditions: 

AFRE may edit the biography included in this communications packet for clarity, length and style. Any edits made to this biography will be shared with you for approval prior to usage in any public-facing materials, including on AFRE’s social media, website, and other forms of media. If you wish to use a different biography, you will send it to AFRE, along with this signed form, by 15 July 2022. 

The headshot included in this communications packet is approved for usage in AFRE’s public facing materials. If you wish to submit a different headshot, you will send it to AFRE by 15 July 2022. 

AFRE engages photographers and videographers to record certain sessions or immersive experiences throughout the fellowship year. You agree to give AFRE permission to use these photographs and videos in its publications, without payment or other consideration. 

You understand and agree that you must obtain permission from AFRE before using or distributing any photos and videos taken by AFRE. 

You authorize AFRE to reasonably edit, alter, copy, exhibit, publish, or distribute videos and photos containing your image or likeness. Should you, by exception, wish to decline to be photographed or videoed, you agree to advise AFRE in advance and to assist with any measures that enable that to happen efficiently. 

You will send all collateral materials (online, print, television, etc.) produced with funding provided by AFRE—or that attempts to represent AFRE’s ideas, pedagogy, or impact—to AFRE for review and written approval prior to usage. 

You agree to ensure posts on your personal or professional social media, online profiles, websites, newsletters, etc. about AFRE, the fellowship experience, or the global fellowship community reflect the Atlantic Fellows’ shared criteria, attributes, and values. 

If you are invited to, or wish to, interview with media (online, print, television, etc.) or to publish content (blogs, articles, social media, etc.) specifically about AFRE, the fellowship experience, or the global fellowship community, you agree to first consult with AFRE. 


STATEMENT OF COMMITMENT TO OUR COMMUNITY 

I, the undersigned, agree that:  


1. I have read and will abide by the Fellows Guide, and my engagement with AFRE and the global Atlantic Fellows network will uphold the mission, values, and goals of the Atlantic Fellows programmes.

2. I understand my role as an Atlantic Fellow is a significant responsibility, and I agree to participate in all required activities. I acknowledge that if I do not fulfill my responsibilities, I will be asked to exit the programme. These includes:
  • Preparing for fellowship sessions by completing reading and other assignments.
  • Setting up my account on AFRE's learning management systems, Canvas, with support from the AFRE Staff, and ensuring active usage of my Atlantic Fellows email account.
  • Committing to find a quiet and private space from which to participate in virtual sessions.
  • Attending all required programme activities, being on time and actively participating for their duration.
  • Showing up for the community by being on video to maximise my ability to connect with other Fellows.
  • Fostering the development of my peers through my participation in dialogues and other fellowship activities.
  • Alerting AFRE Staff in advance of any technological challenges so that Staff may assist in finding a solution.
  • Notifying AFRE Staff timeously before the meeting in case of absence due to illness, unavoidable travel, or family crisis.
3. I understand that I must submit all required paperwork by the deadlines provided for AFRE to arrange travel and other logistics, and that if these deadlines are not met, I am responsible for making and paying for travel arrangements on my own.

4. I understand that the Atlantic Fellows programmes offer a space that brings together people working in different sectors and contexts, from different racial and ethnic backgrounds, and with different ideological perspectives social change strategies. I will bring curiosity, and extend care and respect to all members of the fellowship community.

5. I understand that the primary purpose of AFRE scholarship is intended to cover any virtual participation costs and incidental travel costs associated with my fellowship that are not paid directly by AFRE. I am aware I can use the resources to supplement my leadership development in other discretionary ways too. If during the fellowship term, I cease to attend and participate in required activities, I understand that remaining scholarship payments will be withheld, and I may be asked to return funds already provided to me.

6. I agree not to engage in any conduct that is calculated to, or which has the effect of bringing the name, image, goodwill, and reputation of the Atlantic Fellows for Racial Equity, Columbia University, Nelson Mandela, the Nelson Mandela Foundation, the Atlantic Institute, or the Atlantic Philanthropies into disrepute.

7. I understand that as a Fellow, I am answerable to AFRE, Columbia University, and Nelson Mandela Foundation policies and procedures, including those related to conduct within the programme as well as to discrimination, harassment, assault, and violence. I understand that failure to comply with these policies or any use of hate speech (as codified in South African law) may result in termination of my fellowship.

8. I understand that all collateral, print, broadcast or other material crediting or mentioning AFRE, Atlantic Institute, Columbia University or Nelson Mandela Foundation should be sent to the AFRE staff person handling communications and branding for review and approval prior to usage. Each usage requires approval. 

9. I agree to refrain from sharing my acceptance status until AFRE's public announcement of the cohort, and will ask my friends, family, and colleagues to do the same.
