I, hereby release and assign all copyright ownership of the images taken of me to Susan Baggett and give up all claims of ownership, income and editorial control. I understand by signing this release, I grant the Photographer the right to alter, publish and/or sell the resulting images and waive the right to receive any compensation for use of images as well as give up the right to approve any finished product. The resulting images may be used for promotional purposes, advertising, photography competition, display within or outside of Photographer’s studio, and any other purpose thought proper by Susan Baggett.
I acknowledge by signing this form I agree that all photographic materials, including but not limited to digital negatives, prints and previews are the exclusive property of Susan Baggett. If model is to receive a CD of images by purchase or by gift from Susan Baggett, images are to be used for personal use only. Use of images for any other purpose, written permission must be requested and received by Susan Baggett.
If model is under the age of 18 the parent/legal guardian must also sign below.
If model is too young to read and understand the above model release, the parent/legal guardian will assume responsibility to act on behalf of the model stated above.
